Facilitation and maintenance of chronically disabled ex-psychiatric persons in the community who have difficulty meeting the demands of life.
Baldwin Housing Program: Provides a supportive living environment for 14 individuals.
Community Enrichment Services Program: Offers case management for approximately 65 individuals with chronic psychiatric illness.
The Health Promotion Program: Delivers public education regarding mental health issues and fosters community development.
The Evening Social Recreation Rehabilitation Program: Organizes a variety of after hours social activities.
The Residential Skills Development Program: Delivers service to individuals with chronic mental illness living in second level lodging homes and homes for special care.

HPS is a comprehensive, community-based treatment and rehabilitation agency. The target population is adults with a diagnosis of schizophrenia. The program provides long-term case management and consultation services are available on treatment, rehabilitation and community planning. The program is also involved in research, investigating factors that influence the rehabilitation of persons with schizophrenia and evaluating the effectiveness of the services.

CMPP is a community-based service for individuals with severe mental illness. The service is offered through the Hamilton-Wentworth Department of Public Health and includes long-term case management as well as shorter term counseling/treatment services. In addition, CMPP provides support and education to second level lodging home operators, partnership with the Mental Health Rights Coalition and HPH's Bridges to Discharge Project. A range of other services are also provided offering support to the homeless, sexuality health promotion education, a friendship group at the Women's Centre, a family education and training program and various research projects.

WPOP provides case management/support services to adults who have a serious and prolonged mental illness. Group activity and skill development programs are offered daily to clients of the program. A medication clinic is also provided. Crisis intervention and management are delivered by case managers in consultation with the program.

Serves City of Hamilton
Comprehensive rehabilitation program for young adults (ages 18-35) suffering from psychiatric illness who are living in the community. Program stresses the development of independent living, social word skills as well as effective use of leisure time. Clients attend the program 4 days per week for 14 weeks and participate in a number of structured group and individual activities. Family support, education and a follow-up is also provided.
